Dos & Dont's  

DO call to make sure a check is ready before you come pick it up.

Please DON’T bring the client with you to pick up a check(s)!

DO send receipts signed by the client if you loaned him or her money and need to be repaid. Please be sure to identify the client on all receipts or include a cover sheet if being faxed.

Please DO write in the complete date that you want a disbursement issued or started and be sure that the date is a check run day.

For all payments requested please DO indicate what the payment is for.

DO specify the exact date of the change of address (MO DAY YEAR).

DO list the client’s current physical address on all transmittals with the effective date (MO DAY YEAR).

Please DON’T have the checks made payable to you (unless to repay a small loan). Checks should be made payable to the CLIENT, BOARD & CARE HOME, a COMPANY, or a VENDOR.

DO prorate rent or board & care payment when the client moves after the 1st of the month.

DO write the complete address where a check is to be mailed.

Transmittal Completion  

In order to prevent mistakes or misinterpretations, transmittals WILL NOT be processed unless:
  • The entire top portion of the transmittal is completed, including:
    • Client’s first and last name
    • Social Security Number
    • Current residence address
  • The proper issue dates of checks are indicated
  • The entire address the check needs to be mailed to is given
    • Do not write “same” for address
In addition, if the client has moved the, the entire 2 page transmittal must be completed. This includes:
  • The date the client moved
  • The rental/board and care payment, the payee, and the address which the check needs to be mailed to
  • Spending money amount, the payee, and the address which it needs to be mailed to (even if it’s the same address)
  • All other disbursements such as transportation, phone bill payments, etc., should be given
  • The second page of the transmittal properly and completely filled out
Incomplete or incorrectly completed transmittals will not be processed and will be rejected.
